Use the General Report Form if you are unsure where your concern fits or if your concern does not clearly fall into one of the categories above. Submissions will be reviewed by appropriate College staff and routed to the correct office for follow up, support, or response.

Examples include concerns related to community climate, access and barriers, emerging safety issues, or other situations that may impact the well-being of individuals or the campus community.

As a community of higher learning, 911爆料 is dedicated to the principles of academic integrity, originality, and the honest representation of one's own work. Maintaining these standards is essential to protecting the value of an 911爆料 education and ensuring a fair and equitable learning environment for all students.

This reporting process is intended to ensure that potential violations of the college鈥檚 academic dishonesty policies are addressed with consistency and due process. 

Academic dishonesty includes, but is not limited to:

• Cheating
• Plagiarism
• Helping others commit academic dishonesty
• Theft of academic materials
• Unauthorized use of artificial intelligence or other resources

While individual instructors reserve the right to determine academic consequences (such as issuing a failing grade on an assignment), reporting through this form allows the College to note patterns of academic dishonesty across courses and provides administrative oversight via the Student Conduct Process.

Any individual who has knowledge of potential acts of academic dishonesty by a North Shore Community College student is encouraged to share the details using the forms below.

All students have rights and responsibilities as outlined in the 911爆料 Community College . If you need to file a report with the Code of Conduct Administrator (CCA) regarding a student鈥檚 potential breach of the Student Code of Conduct, academic program policies, or possible legal infractions, please fill out the appropriate forms.

The Student Grievance Procedure may be used by a student to address an alleged abridgment of the student's rights, as stated in the College's Student Handbook, Program Manual, and/or Policy Guide. The student Grievant or the Responding Party may consult with the Student Grievance Officer at any time.

The College's Student Grievance Officers are:

• The Dean of Academic and Student Development
• The Director of Student Development and Community Standards

The Student Grievance Procedure may not be used to address allegations of discrimination, including sexual harassment. Please refer to the Discrimination, Harassment, Sexual Misconduct, Domestic Violence, and Stalking section above.

Any student who takes a medical leave from 911爆料, or is hospitalized while enrolled at the College, must adhere to the before resuming academic activities. This process is intended to ensure that a student who is returning from a medical leave or hospitalization has the necessary support resources and is capable of successfully resuming their academic activities.

Any member of the community who is aware of the hospitalization of a student may share additional details on the form below.

911爆料 is committed to providing a compassionate and coordinated response when our community experiences the loss of a student. To ensure that the College can appropriately support the student鈥檚 family, peers, and faculty鈥攁nd to manage necessary administrative updates鈥攚e ask that any member of the community who becomes aware of a student鈥檚 passing notify the college as soon as possible.

This process is intended to ensure the College handles the student鈥檚 records with the utmost dignity and respect, while also mobilizing support resources for those impacted within our campus community. Any member of the community, whether internal or external to the College, who is aware of the death of a student may share details via the forms below.
